Please call into reception and complete the relevant registration form (or download one from the above "Registering Online" tab).

The practice operates within NG6, NG8 and NG16 and the receptionist will check that you live within the practice boundary.

When you register you may state your preferred GP but appointments with that GP may depend upon availability.

If you are on repeat medications, please bring a prescription slip from your last practice when you register, so our GP can see what medications you usually take. Alternatively, if you bring your tablets to the surgery, the receptionist will take a note of your medication and pass it to the GP to be added to your records. Sometimes, the GP may want to speak to you before prescribing medication on repeat, even if you were taking it at your last practice.
